At OneCare, we have Health Improvement Practitioners and Practice Pharmacists that you can see
Health Improvement Practitioner
A Health Improvement Practitioner can help you to improve your health, wellbeing, and quality of life. A HIP is a registered health professional who has the knowledge and skills to help you with:
1.Stress
2.Sleep
3.Anxiety, depression, or feeling down
4.Coping with physical health conditions
5.Relationship challenges
6.Drug or alcohol concerns.
When you meet with a HIP, you’ll have a chat and can together you can create a plan to start making positive changes right away.
Meetings with HIPs are free, brief sessions, up to 30 minutes. They will focus on practical strategies to support you with your wellbeing, and can link you with external services if you need them. Meetings can be face to face, by phone, or video consults – whatever works best for you.
Ask your doctor or nurse for an appointment with the HIP.
Practice Pharmacist
A Practice Pharmacist is an experienced clinical pharmacist with general medicine skills. They can answer your questions and help you and your whānau understand your medicine(s).
This role is different to the support you might receive from your community pharmacist. Your GP or Nurse may refer you to the Practice Pharmacist when:
1.Your medicines are complex
2.You may be experiencing side effects
3.You have recently been in hospital
4.If there may be other medicines you could take.
The Practice Pharmacist will work with you and your GP or nurse to make sure the medications are right for you before making any changes.
They can help monitor and review your medicines. They are trained to make changes to your medication when it is needed, and make sure everyone involved with your care is aware of your current medication plan.
Practice Pharmacists will work closely with the GPs, nurses and others in the practice team.
The Practice Pharmacist service is free, however, when other practice clinicians are also involved in your consultation there may be the usual consultation fee. Please speak to the reception team if you have any questions.
